How do you make calls over an internet / VoIP connection?

Since the call is made digitally over the internet, the only difference between VoIP and traditional phone service is that an IP phone is plugged into an Ethernet connection instead of a phone jack. Once plugged in, you can pick up the handset and communicate without concern about connection quality or interrupted audio.
